At Oak Hill and Hatfield Hyde we take great care to ensure a high quality and dignified service, you can book an internment of ashes with us directly or through your funeral director.
Send completed forms back to contact-whc@welhat.gov.uk at least three workings days* before.
If the cremation took place elsewhere please also supply the cremation certficate.
If you do not supply all of the information requested we may have to cancel the booking.
*In some cases we may be able to offer an internment of ashes with less than three days notice but this is at our discretion.
